3 BMI Limitations
Body Mass Index (BMI) is a widely used screening tool, but it has well-documented limitations that users must understand before interpreting their results. BMI is calculated from height and weight alone — it does not directly measure body fat percentage, fat distribution, muscle mass, bone density, or overall metabolic health.
BMI results may not accurately reflect health risk for the following groups:
Athletes & Muscular Adults
High muscle mass can produce a BMI in the overweight/obese range even with very low body fat. BMI cannot distinguish between fat and muscle.
Older Adults (65+)
Age-related muscle loss (sarcopenia) means BMI may underestimate body fat percentage. Waist circumference is often a more useful measure for older adults.
Pregnant Women
BMI is not an appropriate health measure during pregnancy due to expected weight gain from the baby, placenta, and amniotic fluid. Please consult your midwife or GP.
Different Ethnic Groups
People of South Asian, Chinese, and some other ethnic backgrounds face higher metabolic risks at lower BMI values. NHS applies lower thresholds for these groups (obese from BMI 27.5).
Children & Teenagers

People with Large Bone Structure
Naturally larger bone frames can inflate BMI without any excess body fat. BMI does not account for skeletal variation between individuals.
A BMI result outside the healthy range (18.5–24.9) does not necessarily indicate the presence of a health condition. Similarly, a BMI within the healthy range does not guarantee that a person is in good health.
